Athletic Waiver and Release of Liability - Please Read before signing
In consideration of being allowed to participate in any way in the Saints Indoor Soccer League athletic sports program, related events and activities, the undersigned acknowledges, appreciates, and agrees that:
All participants are required to assume all risk by signing this general release. I understand that the risks I assume by participating in the Saints Indoor Soccer League (SISL). In consideration of receiving permission to participate in the SISL,
I hereby release the SISL and The Proctor School District (ISD 704), and all its employees from all liability for any personal injury, death or damage of any kind arising from or related to my participation in the SISL. My decision to participate
in the SISL is voluntarily assuming all risk or loss. Damage or injury, including death, which may occur through my participation in the SISL and I understand that by signing this release waiver I am giving up the right for myself and/or my family
or heirs to sue the SISL or ISD 704 and employees for injuries resulting from participation in the SISL. I agree to abide by all RULES and REGULATIONS governing the SISL which includes any disciplinary measures or suspensions levied by the SISL League Coordinators.
I understand that the league has a zero tolerance for fighting and further understand that if I should engage in a physical altercation, I will be immediately suspended for the remainder of the current season, with no refund.
I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES or others, and assume full responsibility for my participation; and,
I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.
